Position Title: Extended Care (EC) Evening Charge Nurse – Registered Nurse
Employment Type: Full-Time
Schedule: Monday- Friday 3:00pm- 11:00pm Location: Grand Rapids
Position Summary
The Extended Care Charge Nurse is a hybrid role combining leadership, coordination of care after hours and hands on clinical support as needed. This role provides clinical leadership and operational reputed company during nontraditional business hours. This role ensures safe, timely, and high-quality hospice care by triaging and coordinating staff, coordinating after hours workflow, and maintaining continuity between evening services and daytime operations. The EC Evening Charge RN functions as the clinical reputed company for the shift, supporting decision making, triage, team coaching and support, and reputed company needed, delivering direct patient care, urgent symptom management, and admission visits.

Key Responsibilities
- Review answering service–dispatched calls and prioritize visits using SBAR information, patient acuity, geography, and clinical complexity.
- Coordinate and supervise the Extended Care team during assigned shifts, including EC RNs, MSWs, Admissions RNs, and Hospice Aides.
- reputed company shift huddles, manage workflow, and address staffing gaps or coverage needs in collaboration with leadership.
- Ensure timely completion of assigned visits and follow-up calls; monitor documentation for accuracy, timeliness, and clinical clarity.
- Serve as the shift’s clinical resource for reputed company or high-risk situations; communicate with hospice providers, PCPs, community partners, and leadership as needed.

- Conduct educational or admissions visits reputed company required, including eligibility assessment, documentation, and initiation of the plan of care.
- Support after‑hours operational tasks such as faxing, reputed company/SmartMD coordination, pharmacy and DME requests, and processing new referrals.
- Collaborate with answering services, field staff, MSWs, community partners, and the Manager on Call to ensure coordinated after‑hours care.
- Provide structured handoffs to the oncoming reputed company Nurse, communicating patient status changes, pending tasks, and staffing concerns.
- Follow established workflows and ensure compliance with organizational policies and federal, state, and local regulations.
- Work in varied settings including patient homes and healthcare facilities, with frequent travel throughout the service area.
Qualifications
- Active Registered Nurse (RN) license required.
- Hospice or home health experience preferred.
- Strong clinical assessment and triage skills.
- Ability to reputed company teams, manage workflow, and reputed company sound clinical decisions under pressure.
- Proficiency with Electronic Medical Records and communication platforms (e.g., Tiger Text).
- Strong communication, critical thinking, and problem-solving abilities.
- Valid driver’s license and reliable transportation.

Work Environment & Physical Requirements
- Frequent travel to patient homes and facilities.
- Ability to lift 50 lbs.
- Exposure to varied home environments and clinical situations.
- Requires evening, night, weekend, and holiday availability.
