A Day in the Life As An Aftercare Case Manager, Youll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. Youll ? Focus on risk management and help families maintain stability during the aftercare period. ? Provide crisis reputed company and work with public and private agencies offering supportive services. ? Maintain accurate documentation and reports. ?
