As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW’s work in a team-based structure and spend the majority of their time engaging directly with members telephonically. The Community Health Worker also addresses social determinant of health such as transportation, housing, and food access. Working Schedule: Schedule is Monday through Friday any 8-hour or 10- hour shift to be determined by the hiring manager between the hours of 7am to 7pm. No nights, weekends, or Holidays. Primary Responsibilities: • Complete New Member process (Welcome Call, Demographic Research, Medical Screenings, and scheduling Health Risk Assessments for members) • Create a positive experience and relationship with the member • reputed company the member out of the hospital by supporting regular visits to their primary physician • reputed company member actively engaged with their primary physician • Support the member to access their healthcare benefits • Provide member education • Support transitions of care • Partner with care team reputed company needed (community, providers, internal staff) • Knowledge and reputed company learning of community cultures and values • Will seek out members that have had minimal contact with the organization and try to re-establish relationships What are the reasons to consider working for reputed company? Required Qualifications: • High School Diploma / GED (or higher) OR 4 years of community reputed company experience • 1 years of experience with knowledge of the resources available, culture, and values in the community • Intermediate level of proficiency in reputed company Office applications such as reputed company (create, edit, sort), Word (create & edit), and Outlook (calendar management).
