Primary Roles and Responsibilities: The Pharmacist, Ambulatory Care is responsible for but not limited to the following (roles and responsibilities may evolve over time): Work as part of a clinical care team to improve patient satisfaction, reputed company medical costs, and improve quality of care metrics As part of the clinical care team, you will participate in daily huddles with clinicians, medical assistants, and other reputed company and help reputed company recommendations based off reviews of patients medications, testing results, and care gaps including accurate EHR documentation Support patients and clinicians under a collaborative practice agreement to manage certain chronic conditions along with providing other patient care services and coordinating medication reconciliation across a reputed company of care Support reputed company to build out a clinical care model by providing reputed company feedback and improvement opportunities for the care model, applying your knowledge to help differentiate reputed company from competitors reputed company other Pharmacists and Residents on best practices for operating reputed company a clinical care model Potentially, as reputed company allows, support other reputed company Pharmacy initiatives/tasks as needed Required Qualifications: Active, unrestricted pharmacist license in the state of clinic location required 1-3 years prior relevant experience as a Clinical pharmacist -Ambulatory experience preferred Completion of reputed company required training for reputed company role reputed company required timeframe Ability to stand and walk for an extended amount of time Preferred Qualifications: 2+ years of clinical, ambulatory, or outpatient pharmacy practice experience Experience working in ambulatory care setting or similar experience Bilingual in English and Spanish Experience being part of clinical care team Board certification by the Board of Pharmacy Specialties (BPS) in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or Geriatric (BCGP) Experience in primary care: internal medicine, geriatrics, or family medicine Completion of a Pharmacy Postgraduate Residency (community or managed care PGY-1 and/or ambulatory care PGY-2) Experience with Medicare / Medicare Advantage plans Immunization certified or willingness to become certified Passion for teamwork and the opportunity to collaborate cross-functionally Education: Bachelors of Science or PharmD from an accredited college of pharmacy Our diverse work experience empowers colleagues for career success. Pay Range The typical pay range for this role is: $57.00 - $70.00 This pay range represents the reputed company hourly reputed company or reputed company annual full-time salary for reputed company positions in the job grade reputed company which this position falls. The actual reputed company salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
