Videos Duties The Home Telehealth nurse is a professional nurse who monitors patient status and coordinates care across the healthcare continuum for high-risk patients/high utilizers with certain chronic conditions. Responsibilities include coordination of care, disease management education, medication management, reinforcement of the clinical treatment plan. This nurse plays an integral role in helping to reduce unscheduled ER visits, unscheduled walk-in clinic visits and helps to reduce bed days of care by teaching patients the self-management skills that are needed to stay well.
